Process a hook invocation for Kiro (IDE and CLI).
Kiro’s PreToolUse hook receives JSON on STDIN with tool_name and
tool_input. The hook outputs the modified tool_input JSON to STDOUT.
Exit code 0 = allow (proceed with modified input).
Exit code 2 = block.
Kiro uses execute_bash (alias shell) as the tool name for shell
commands, with tool_input.command containing the command string.
